To whom it may concern I can confirm that my patient has had a stroke and been left with residual neurological deficit. As a consequence, he has the following difficulties. He can only stand very briefly and does not have the strength to cook or cut up food. He is unable to get in or out of a bath or shower unassisted and can not stand without help. He needs assistance to manage his toilet needs and is not able to dress or undress the lower body because of his condition. He is not able to read written information and needs assistance in journeys due to anxiety and distress. Severe osteoarthritis in both hips means he is unable to stand without pain and cannot walk more than 20 metres unaided. This is consistent with my knowledge of the patient. In addition, he has a history of Hypertension, Hyperlipidaemia, Conn’s syndrome, Anxiety and Depression, and Diabetes Mellitus. A copy of his medications is attached.
